Any location, place, building, resort or space, whether covered or uncovered, which is used to operate or conduct or maintain any type of show, concert, lecture, go-go dancing, entertainment, cabaret, performance, exhibition, amusement or resort or a place of entertainment, amusement or resort or public entertainment which is open to the public or to which the public is invited or may enter, including premises uses as a clubhouse or club room.

Information required with the amusement license application:
- Three passport size [2x2] photographs of the applicant.
- Copy of valid driver's license, or other proof of identification.
- Copy of applicant's social security card.
- State Sales Tax Certificate of Authority
- You must have zoning approval.
- Proof of ownership or leasing (copy of deed/lease).
