Fees: Coin-controlled amusement devices
   Pinball or Electronic (each) = $415.26
   Pool Tables (each) = $415.26
   Transfers (each) = $21.16

Coin-controlled mechanical devices
   Annually = $31.74

Coin-controlled music device
   Jukeboxes (each) = $415.26
   Wall Units (each) = $10.58
   Transfers (each) = $21.16

Information Required with the Coin Controlled License Application:

1. Two passport size [2x2] photographs of the applicant.
   
2. Copy of valid driver's license, or other proof of identification.
   
3. Copy of applicant's social security card.
   
4. State Sales Tax Certificate of Authority
